Elderly Community Care Vouchers

Service Introduction

The Social Welfare Department (SWD) launched the third phase of the "Elderly Community Care Service Voucher" pilot scheme in October 2020. Under the "money follows people" model, eligible elderly people can use community care service vouchers (CCSVs) to choose appropriate community care services according to their personal needs to support their home-based ageing. The pilot scheme covers all 18 districts in Hong Kong, and the maximum number of CCSVs can reach 8,000.

Eligibility

Elderly persons who have been assessed and recommended as suitable for community care services or residential care services under the "Uniform Assessment Mechanism for Elderly Care Services" and are currently on the Central Waiting List for Subsidized Long-term Care Services for autonomous community care services and/or residential care services, but have not yet received any residential care services or autonomous community care services.

1. 65 years old or above; and
2. Assessed and recommended by the Social Welfare Department as being suitable for community care services or residential care services; and
3. Currently on the Central Waiting List for subsidized care services; and
4. Elderly persons who have not yet received any residential care services or subsidized community care services

How to apply for a community voucher?

1. Applicants or their family members need to first apply for the Community Voucher Scheme at the Medical Social Services Department, the Integrated Family Service Centre or the Elderly Service Unit in their respective districts.

2. The social worker or staff in charge will conduct an assessment for the elderly.

3. Elderly persons who are assessed to have moderate or severe disabilities will receive an invitation letter from the Government to participate in the Elderly Community Care Service Voucher Scheme (CCSV).

4. Complete and return the relevant forms.

5. Your application is successful once you receive the "Community Voucher Certificate".

The Elderly Community Care Service Voucher Scheme provides community care services in a funding model of "money follows people" and "those who can afford it pay more".

Features
"Money follows people": Directly subsidize eligible elderly people, who can freely choose and switch approved service units, service items and service volume under the Community Care Voucher Scheme.

"Those who can afford it pay more": Allow elderly people with lower affordability to receive more government subsidies.

"Mixed model" service: Elderly people can use "day care center services" and "home care services" at the same time.

Services can be obtained in a short time: Generally, the Social Welfare Department will complete the review and issue of community vouchers within 2 to 4 weeks after receiving the application.
